Duplicate copies will be sent to our email address and your email address.
I formerly request Affiliation with the Missionary Agency of World Outreach Ministries, Inc. and give permission for the application process to move forward.
I understand that I will be an "Affiliate missionary" operating as an independent mission worker (not an employee). World Outreach Ministries Inc. does not ordain or send workers to the mission field – or assign duties for you to perform on our behalf. The mission work that you undertake and the risks that you take are your own responsibilities. "With this understanding, I am requesting WOM's administrative help through their Missionary Service Agency. I have read the Policy Manual, and I am in agreement with it."
Further, I understand that the offer for Affiliation with the WOM Missionary Agency is contingent on a background check for USA citizens and residents (for non-USA citizens we will work with you via a different process). I will complete the DocuSign forms that will be emailed to me, granting permission for the background check. (The forms will be emailed to you and only take 2-3 minutes to complete and submit online – no printing or scanning involved). I also understand that I may withhold my permission – and in such case, the background check will not be completed and my Affiliation with the Missionary Agency will not be processed.

USA Address - Background forms vary from State to State. If you are a USA citizen or resident, please enter the USA address that you use on your Tax forms – or your last USA address.
